Hello. I created a reading log form for my class so that the results could be added up in a spreadsheet and divided and sort by the month that they read their books in.  However the formula I am using does not seem to work for every and and I have no idea why.  The spreadsheet nis at :

I am trying to get it so that all the results go into the "main" page, and then are sorted into the corresponding month page and added together.  But something doesn't seem to be working for all the entries. For example, on the October sheet, Jessi's name is twice in a row in column d, and I have no clue why. It should only lookup their name and sort once.  Any help would be greatly appreciated. Thanks.

Hi again. I am helping a friend with this and she got your email about sharing the spreadsheet.  I worked on this a little bit and got the column D working okay but for some reason column B is not working for 2 of the students and I have no clue as to why.  I think you helped me with this about a year ago on these forums so this should look somewhat familiar :) Thank you so much.

one of the problem is that some student names look similar but are different because they may have some spurious space characters in them ... 
in any event the following solution to the problem should work correctly
